Transformational Housing Case Manager

The Front Door Agency, Inc.(FDA) is seeking an experienced, creative and high energy Case Manager to build on past program successes and help take its transformational housing program to the next level of effectiveness. For over 31 years, the Front Door Agency, Inc. has offered support and programs to assist families and individuals transition from crisis to self-sufficiency. It is located in Nashua, New Hampshire and has a great reputation of serving families at-risk of homelessness in becoming independent through its Transformational Housing, Housing Stability, Affordable Housing, and Financial Literacy Programs.

Reporting to the Transformational Housing Director, the Phase I Case Manager will be a member of the FDA’s transformational housing team. S/he will provide extensive case management services to mothers and children who are experiencing crisis, but who are motivated to break the cycle of poverty and become independent through education. The Case Manger will work closely with prospective and current clients in providing direction, guidance, establishment of goals, and creating accountability.

S/he will lead in-service coordination and determine appropriate services/referrals needed; coordinate life skills groups; assist in personal budgeting; ensure client compliance with program and housing rules and expectations; document outcomes; and maintain a professional working relationship with staff, community and clients. The key role of the position will be to create a positive environment for mothers and children as they begin to rebuild their lives while also helping them to meet personal and program goals. S/he will be working in partnership to help mothers and children stabilize, transform and ultimately become empowered to reach self-sufficiency.

The ideal candidate will have a passion for the mission of the Front Door Agency, a keen understanding of its programs and goals, and the ability to articulate the important role the organization plays in the lives of so many families in the Greater Nashua area. The ideal candidate will have a Bachelor’s Degree in Human Services, Psychology or similar field; a minimum of three years of direct case management experience; be proficient in Microsoft Office Suite particularly Excel; and have some knowledge or experience working with women who have mental health or substance use disorder histories. S/he must possess strong verbal and writing skills, and must have a collegial, team-building style and be able to lift 20 lbs.

The position is considered full time Monday- Friday. Salary commensurate with experience.

Residential Aide – Part Time

The Front Door Agency is looking for a part time Residential Aide to join its team helping women and children served in its Transformational Housing Program. If you are looking for a rewarding part time position that can make a difference to others in a relaxed atmosphere, this may be the position for you! In keeping with the goals and mission of helping women and children stabilize, transform and ultimately flourish to independence and self-sufficiency, this position is a key member working evenings, weekends and overnight (non-awake) and some holidays to ensure families are in compliance with housing related rules and to help support case manager’s resident goals and plans in her absence. Applicants should be positive, responsible, professional, exercise good judgment and be willing to complete a criminal background check and be able to lift 20 lbs. Specific Job Responsibilities Include:
• Maintaining an orderly and positive environment
• Ensuring all residents are in compliance with the residential component of the program
• Supporting the Case Managers in carrying forth resident goals/plans in their absence
• Working as a team to ensure resident success by being available to help them meet their needs
• Being available to residents and children to guide them towards meeting their individual goals
• Greeting and monitoring all guests
• Accepting and sorting donations
• Setting up a welcoming environment for incoming families
• Maintaining confidentiality
Qualifications and Skills
Associates degree in related field preferred or a minimum of five years’ experience in the human service field.
Hourly Rate:  $10-$12 an hour
